How do I get the wire to form a perfect circle?

An oval wire is caused by over-closing the binding around your book. To fix this, just adjust the wire closer diameter to reduce the pressure to close. Usually this is done with a knob or dial. Your machine should have a closer setting for each different size wire, so it is possible that it was set to the incorrect size. Or if it is on the right size, perhaps the machine needs some fiddling after a lot of use.

Why are pages are falling out of my book?

Pages will fall out of your wire bound book if the wire is not closed enough. The fix is the same as above: to adjust your wire closer, but in the opposite direction, setting it tighter for smaller wire.
